INSTRUCTIONS TO BE READ BEFORE COMPLETING THE CLAIM FORM
Before you fill out the online form, make sure you have the following documents on hand:
1) A copy of a government-issued photo ID so we can confirm your identity. This could be your health insurance card or driver’s license;
2) A proof of residence at the eligible address for the period between 2009 and 2018,
OR
A sworn statement explaining why you have no proof of residence AND specifying the dates of residence for the eligible address(es) where you have lived.
You will find a sworn declaration template on our website. You can then contact TJL to be sworn in.
IMPORTANT: all residents of an eligible address must submit their own claim form.
Therefore, 2 residents = 2 claims.

You must submit your claim NO LATER THAN AUGUST 27, 2024.
